An attacker with Ostium's oracle signer key submitted a future-dated price report through the PriceUpKeep Forwarder, looped ~20 delegated trades, and drained roughly $24M from the OLP vault. TVL fell from $32.7M to $9M. The smart contracts did exactly what they were told.
Verus Redeposited $11.56M Into The Bridge That Just Got Drained. Two Weeks Later An Attacker Emptied It Again For $7.54M
Verus refunded its bridge on July 8 after the May exploit. Fourteen days later a different attacker drained $7.54M through the same forged import path bug class. Blockaid flagged the entry vector as identical. Post-incident review missed the class.
AFX Trade Called Its Bridge A Multi-Sig. Five Hot Keys Signed $24.15M Of USDC Out In One Transaction
AFX Trade lost $24.15M USDC after five hot validator keys cleared the bridge's 2/3 quorum and authorized a single transfer. Funds were bridged to Ethereum and swapped for roughly 12,467 ETH. Arbitrum's native bridge was untouched.
